("**Producer**"), desires to record and use the performance, likeness, voice, name and other personal characteristics and/or information of the individual named below ("**Performer**"), and to use other media made or supplied by or on behalf of the Performer (“**Content**”),  to appear in audio, photo, and video productions and for advertising, promotion and other commercial purposes. In consideration for participating in the recording, and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, Performer hereby gives Producer permission for such use, according to the terms in this agreement ("**Agreement"**).

**THIS AGREEMENT PROVIDES THE PRODUCER WITH AN ABSOLUTE AND UNCONDITIONAL CONSENT, WAIVER AND RELEASE OF LIABILITY, ALLOWING THE PRODUCER TO PUBLICIZE AND COMMERCIALLY EXPLOIT THE PERFORMER’S NAME, LIKENESS, PERSONA, VOICE AND OTHER PERSONAL CHARACTERISTICS AND INFORMATION AS SET OUT ABOVE. BY SIGNING, YOU ACKNOWLEDGE THAT YOU HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS WAIVER AND RELEASE AND THAT YOU MAY BE GIVING UP SUBSTANTIAL LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE THE PRODUCER.”
